Our office is located at 6711 Forest Lawn Dr., Ste. 104 Los Angeles 90068, inside the Toluca Lake Tennis Club. The Toluca Lake Tennis Club is the fourth building east of Barham Blvd. on the north side of the street. It is white with a red tile roof and green fences around the tennis courts. Our suite is located at the east end of the building. The Tenant Parking lot is also at the east end of the building. Street parking is usually available but please read the signs before parking. We have 3 tandem spaces for our office (Suite 104). If you want to park in our spaces drive down the driveway marked for “Tenants Only” on the east side of the Toluca Lake Tennis Club. The 3 tandem spaces are in the middle of the lot, on the right hand. Please pull all the way up to the wall or park behind another car. If you are behind another car, please give your keys to us once you come into our office. Once parked, walk toward the building and enter through the double glass doors on the same level as the tenant parking lot and our office, Suite 104, is mid-way down the hall on the right-hand side. Please note, if you are coming to the office for a Saturday appointment, you may park anywhere in the Tenant Only parking lot. On Saturdays please don’t block another car since it’s used by the entire Toluca Lake Tennis Club on the weekend.
OFFICE POLICY REGARDING APPOINTMENTS:
When making your first appointment our receptionist will ask you for a credit card to hold your appointment. We reserve an hour and a half for your first visit, which consists of a consultation and treatment. We ask for the credit card to hold your appointment because this time has been reserved especially for you. We require 24 hours cancellation notice to avoid a missed First visit fee equivalent to the first visit consultation and treatment fee. We will keep your credit card in a secure location and will not charge it unless less than 24 hours notice is given or you do not show up for your appointment.
For returning patients we reserve time especially for you, which prevents others from receiving treatment at that time. Therefore, 24 hours cancellation notice is required or a missed appointment fee of $55.00 will be charged. For the time being we are not charging our entire fee for missed appointments. We charge a portion and in essence share in the cost of a missed appointment. We hope you understand this shared cost is our way of respecting our time as well as acknowledging things do come up and occasionally get in the way of an appointment.
Missed appointment fees apply except in the case of an emergency.
In the State of California acupuncture visits or a portion of treatments are often covered by your insurance. Coverage varies and you should telephone your insurance carrier to find out to what extent you are covered. We will also call your insurance carrier and provide them with the necessary information to find out about your particular coverage. If you are covered we will bill your insurance directly or provide you with a separate bill if you prefer.
